Vehicle Information Form – This form is used to provide SAM the information about the vehicle if the information is not provided on a repair order or estimate. Service Center Deal Funding Checklist - The service provider uses this checklist to make sure he has all the documents required to get funded. Overnight all the documents to SAM when the work is done and the checklist is completed. Interview Acknowledgement Form - Before a deal gets funded we must interview the customer. This form is to advise us as to when the best time is to call the customer. Authorization For Scheduled Loan Payments - If the customer has a debit or credit card they can use it to make their monthly payments. On Time Visa Set Up - If a customer does not have a debit or credit card they must complete this application. We will provide them with a debit/credit card they can load money on. The fees for this card are on the application. If the customer has direct deposit available from his employer there is no charge for direct deposit. |
